IT/ELECTRONIC PATIENT RECORDS
Did you know that Water Meadow Surgery provide/offer the following IT developments:-
On line appointment booking: This provides the ability for patients to book appointments online.
On line booking of repeat prescriptions: This provides the ability for patients to request repeat medications online.
Patient Access to records: This provides patients with access to their medical record online. At the moment this is only access to medications, allergies and adverse reactions and coded information.

Before you can do any of these, we need to issue you with a PIN code and Access ID. To obtain these, please ask at reception next time you visit the surgery - we cannot issue these over the phone or by post.
You can complete this form and bring it along with two pieces of ID; one of these must be a photo ID e.g. passport or driving licence, the other must be something confirming your home address e.g. utility bill.

Summary Care Record: Summary Care Records provide healthcare staff with faster, easier access to reliable information about you to help with your treatment; especially in an emergency. Visit http://systems.hscic.gov.uk/scr for more information.
GP2GP transfers: GP2GP enables us to transfer the Electronic Health Record (EHR) of a patient securely and directly to a new practice when the patient registers at that practice.
